Applicant hereby acknowledges that it has read and agrees to the credit terms and conditions contained herein, including the Additional Terms (attached and initialed) annexed hereto and collectively "Terms", including subsequent modifications thereto. Applicant certifies that the above information and any attachments hereto are true and furnished for the purpose of inducing United Metro Energy Corp. (UMEC) to extend credit to the undersigned.
Applicant agrees to the Terms disclosed in any credit or purchase agreement subsequently issued and that any purchases shall constitute acceptance of those Terms. Applicant authorized UMEC to receive information regarding its banking relationship from the above‐named financial institution.
Applicant authorizes UMEC to investigate Applicant's credit record, obtain credit reports, verify assets, debts, references, income information, and report same to a credit reporting agency and to answer any questions regarding Applicant's credit history with UMEC. If UMEC refers the account to an attorney or collection agency, Applicant agrees to pay all reasonable attorney's fees, court costs, and costs of collection.
Applicant agrees that a reasonable hourly fee or one‐third contingency fee shall be a reasonable attorney fee in the event any matter is referred to an attorney or collection agency for collection. The above authorizations shall be continuing in nature. Upon request, UMEC will inform the Applicant of the name and address of the credit reporting agency which furnished the report
